immediately. FTE may contact my former employers for references as well as conduct a Criminal Background
Search. FTE may release all information received and contained on this application to FTE clients for
employment screening purposes. I do hereby release FTE and their representatives from all liability and
from any damage resulting from release of my information.
I understand that I will be employed solely by FTE on temporary assignments for FTE clients. I agree
that I will work for FTE for 60 days before discussing permanent employment with FTE clients. I also
understand that assignments are for no specific period, and can be terminated at any time without notice
or cause. I will notify FTE when each assignment ends. Failure to notify FTE will result in my not being
called for new assignments.
Foothills Temporary Employment will provide a working environment free of any kind of harassment.

harassment. Should the harassment be determined to have occurred, the appropriate disciplinary action will
be taken. Disciplinary action may include immediate termination of the offending associate and possible
legal action. If you feel you are being harassed on your job, please report the incident to Foothills
Temporary Employment immediately.
